Workshop Series ​

The online workshop series “Accelerating Innovation in Transitional Care – Identifying Key Needs & Meaningful Impact”, designed to engage healthcare providers, experts, innovators, and stakeholders in co-developing solutions that enhance patient care during transitions from hospital to home.

Training Programs

The two training programs — one for Living Labs on service design and commercialization, and one for HealthTech innovators and researchers on navigating the journey from lab to market — aim to equip participants with essential skills for successful innovation.

Project Deliverables

Publicly available reports showcasing the EVOLVE2CARE implementation processes, methodologies, and results, — providing valuable insights for researchers, policymakers, and innovators working to advance Tansitional Care.
